Personal Development Mapper

The Personal Development Mapper helps you reflect on patterns in your experience and possible areas for development. It is not a diagnosis, psychotherapy or a crisis service. This version is for people aged 18 or over who ordinarily live in the UK and are in the UK while completing it. Your answers are used to calculate your results and determine whether developmental feedback is appropriate. AI helps draft the interpretation. Matthew reviews every report before it can be sent. Some answers may mean that a report needs further review or cannot be provided. This does not amount to a clinical assessment of your safety.

Before you begin

Responses are not continuously monitored. Do not use this questionnaire to request urgent help or wait for a response if you need support now. Read the privacy information before answering. It explains how your answers are used, who can access them, the use of AI and how long information is kept. If serious and immediate danger comes to a human reviewer’s attention, they may need to consider sharing limited information to protect someone. Any such decision is considered individually; submitting an answer does not automatically trigger emergency contact. If you or someone else is in immediate danger, call 999 or go to A&E. Do not wait for a response from the Map service. For someone to talk to, you can call Samaritans free on 116 123, day or night.

A little about you

We only ask for information that helps us provide and communicate your Development Map.
First Name(Required)
Email address(Required)
We'll use this to contact you about your Development Map.
Your age range

Please describe your experiences without naming other people or including details that could identify them. Avoid unnecessary identifying details about yourself, such as addresses, phone numbers or workplace names.
A few sentences is enough.

Wellbeing & Suitability

Have you recently had intense spiritual, mystical or altered-state experiences that have been difficult to integrate into everyday life?(Required)
In the past few weeks, have you had a period when you needed very little sleep and at the same time felt unusually energised, driven, impulsive or unable to slow down?(Required)
Is alcohol or drug use currently making it difficult for you to remain safe, function day-to-day, or make decisions you trust?(Required)
